Could someone, preferably a coder, tell me if I'm wasting my time
boosting my spirit, as a Liman surgeon? I know Londoners will be better
at it than me, but do I get any kind of bonus for having spirit above
the minimum reqs?

You'll gain benefits to other liman skills, but surgery, currently,
no.
